The Washington State mandatory family law forms shall be used except where a mandatory form is designated “optional”, local forms have been promulgated by the Court or no mandatory form exists for the particular matter. State and local forms, including Notice of Court Date forms, may be obtained from the King County Superior Court Clerk, the King County Facilitator’s Office, the King County Superior Court Law Library, or by accessing www.kingcounty.gov/courts/clerk/forms.
