
PPE and Medical Supplies Request Form
Updated information on PPE availability
Effective February 23rd, the King County Office of Emergency Management will be limiting PPE to facilities that meet the criteria below:
- PPE supplies are available to organizations and facilities that (1) serve priority populations, (2) currently are experiencing extraordinary circumstances such as an outbreak, and (3) unable to source PPE in a timely manner. This includes the following:
- Critical “safety net” organizations: primarily non-profit organizations such as homeless service providers and community health centers; also, may include faith-based organizations, food banks, other health and human service agencies.
- Certain for-profit Long-Term Care (LTC) entities: Adult Family Homes who accept Medicaid Clients, Supported Living providers, Behavioral Health Inpatient Facilities, and all Assisted Living providers with a Medicaid contract who have memory care units.
- Settings experiencing COVID outbreaks (as confirmed by PHSKC; this is regardless of type of facility) who are unable to quickly purchase enough PPE to manage immediate impacts of the outbreak.

Please use this form to request 14 days’ worth of supplies for your facility. These orders will be transmitted to the state and filled to the extent possible. Once an order is filled, even partially, or if we are unable to fill your order, we will contact you and close the request for the week. You will need to resubmit a request for any unfulfilled orders.
