Please use the form below to report a violation of our vaccination verification order.

Examples of violations include:

  • Not checking vaccination at public events and establishments, including indoor restaurants, bars and taverns and indoor entertainment and recreational establishments
  • No visible signage reminding people that vaccination verification is required at establishments covered by the order

All complaints will be followed up with a communication with the establishment that may include onsite inspections.

Employers, their employees, and volunteers, must implement this Order equitably and may not discriminate based on an individual's race, national origin, religion, or age. Businesses and venues should provide reasonable accommodation for those who are unable to provide proof of vaccination due to a disability.

Sometimes there are chains of food establishments with the same name (i.e. McDonalds) or similar names to other restaurants so to ensure our inspection team is sent to the one you are writing about, we need to know its specific location. Items marked with an asterisk (*) are required.
