For King County Metro Access Transportation

Healthcare professionals play an important role in supporting applications for King County Metro’s Access Transportation Program. As part of the application process, applicants are required to submit professional verification of a diagnosed disability. Some applicants may also require verification for Hand-to-Hand service.

    After an applicant submits their Access application, you may receive a verification request by email.

  • Complete the verification

    Complete the verification using the secure online form or submit a paper form by fax or mail.

Ways to submit professional verification

Complete verification online (recommended)

The online option is the fastest and most secure way to complete professional verification.

After an applicant (or their advocate) requests verification, you will receive an automated email with a secure link to the appropriate form. You can complete the form directly online—no downloads or attachments are required.

How to identify a legitimate verification email

Verification request emails are sent automatically by a third-party provider used by King County Metro Access Transportation Program.

You can confirm the email is legitimate by checking the following:
