Destruction Forms

For destructions performed in-house by your agency or an external vendor, use:
  • Agency Destruction Form (Word, 36KB)
    * Note, this form is not required for the destruction of Transitory Records or Secondary Copies. You may destroy those yourself with no documentation.

For records sent to and destroyed by the Records Center, use:

  • Records Center Destruction Form (Word, 28KB)
    * This form is required for all records (including secondary copies or transitory records) destroyed by the Records Center.

These forms will need to be signed by your Disposition Authority.

Archival Records Transfers

King County Archives accepts transfer of records that have met their legal retention requirements and are appraised by the County Archivist as archival or potentially archival. If you have any questions about the appraisal or transfer of archival records, please contact the Archives at archives@kingcounty.gov or (206) 263-2480.

Destruction After Digitization (DAD) - aka Scanning and Tossing

The Destruction After Digitization (DAD) form outlines the minimum requirements that state and local government agencies in Washington State must meet in order to lawfully destroy paper-based source records after they have been converted to a digital format by scanning. These requirements and guidelines promote best practices for ensuring access to, and retrieval of, digital images throughout their legally-required minimum retention period(s), and assist King County agencies with complying with records retention schedules; King County policies; RCW 40.14, Preservation and Destruction of Public Records; WAC 434-662, Preservation of Electronic Records; and WAC 434-663, Imaging Systems, Standards for Accuracy and Durability.
