Complete this form and submit it along with a plan review application and other required information prior to beginning construction on a new pool or renovating an existing pool. Plan guides no longer need to be submitted. - Construction Report of WRF
Upon completion of WRF construction, modification or alteration and before use, owners shall submit to the health department a construction report signed by an engineer or architect certifying that the construction is substantially in compliance with approved plans and specifications and related to conditions under subsection (2)(c) of this section. - Injury Report Form
The owner or operator of a swimming pool or spa pool must report any death, near drowning or serious injury to Public Health's Environmental Health office within 48 hours of the incident by calling (206) 263-9566.
