- Forms:
- All employees on taking military leave must submit a copy of the orders for training or drill, with their absence request form
- All employees who are activated for military service must provide Human Resources with the following signed forms prior to each activation:
- A copy of your activation orders.
- A copy of the completed Uniformed Services Leave Form for each activation.
- If you do not already have direct deposit, a direct deposit form must be submitted prior to activation.
- If you want to make any changes to your withdrawals, please submit a new W-4 form.
- If your activation will keep you from work during the next Wellness Assessment period, please submit a Wellness Assessment Exemption form to exempt you from the process.
- For all active military leave of absences, an up-to-date Power of Attorney form is needed.
- Please read, sign, date and return a Employee Notification and Acknowledgement form for each activation.
