Accident or fatality

Reporting a serious situation

We care about the safety and well-being of employees, and encourage them to seek medical assistance for a serious injury immediately. Employers are responsible for ensuring reports are made within eight hours of an incident, or within eight hours of learning about it.

A serious injury or fatality is one which results in a workplace death that happens within 30 days of an incident, a potentially fatal workplace injury, or an in-patient hospitalization related to the workplace that happens within 30 days of an incident.

To report a serious injury or fatality:

  1. Call 911 if appropriate
  2. Contact Safety and Claims immediately: 206-477-3350
    After hours: 206-296-3311
  3. If you are unable to reach Safety and Claims contact the Department of Labor and Industries directly: 1-800-423-7233 DLI Hotline*

*Unless moving fallen equipment is necessary to remove any victims or prevent further incidents and injuries, do not move the equipment until a representative of the Department of Labor and Industries investigates the incident and releases the equipment.
